N.J. Admin. Code § 13:33-8.5 - Replacement licenses

If a licensee's license has been lost or destroyed, the licensee may request that the Board provide a replacement license. Such request shall be in writing, include the license replacement fee pursuant to 13:33-8.8 and the licensee shall certify on a Board-provided form that the license has been lost or destroyed and has not been given to another person.
