N.J. Admin. Code § 5:14A-4.15 - Name and address for official correspondence
(a)
The owner of an amusement ride shall maintain a name and address within the
State of New Jersey, or designate the name and address of a representative
within the State, for the purpose of accepting official correspondence from the
Department.
1. Such address shall not be a
post office box, but shall specify a physical location where such owner or
representative may be found during normal business hours.
2. Where the owner or representative is a
corporation, partnership, or other business entity, the name and address of an
officer, or other responsible person, to whom correspondence is to be
addressed, shall be provided.
3. It
shall be the responsibility of the owner to notify the Department of any change
in the identity, address or phone number of the owner or representative. Any
change shall be reported to the Department in writing within 30 days of the
change.
