assessed as to his/her Level of Functioning and Environment/Natural Support
Systems. These assessments, in addition to the client's perceptions of his/her
needs and emotional distress, should serve as bases for the development of a
comprehensive Individual Service Plan (ISP). (See N.J.A.C. 10:37-6, Article
