(b) Except as
otherwise provided in the Rehabilitated Convicted Offenders Act (2A:168A-1 et seq.), no license
shall be issued to any applicant or licensee who has been convicted of forgery,
embezzlement, obtaining money under false pretenses, extortion, criminal
conspiracy to defraud, crimes against the person or other like

licensure as an operator of a group home, supervised apartment, or supported
living arrangement shall be submitted to the appropriate Regional Assistant
Director's Office of Community Services.
The application shall be composed of the following:
i. A table of organization;
ii. Curriculum vitae for the applicant and

provided in (b) above;
