N.Y. Comp. Codes R. & Regs. Tit. 21 § 10051.7 - Requests for records and information
(a) All requests made pursuant to this Part
shall be made in writing and must be accompanied by reasonable proof of
identity pursuant to section
10051.4 of this Part.
(b) A request shall reasonably describe the
records and personal information sought. When possible, the data subject should
supply identifying information that assists the authority in locating the
information sought.
