N.Y. Comp. Codes R. & Regs. Tit. 3 § 401.4 - Change of location
A licensee may, subject to the superintendent's finding that there is no reasonable objection, change its location by giving written notice to the superintendent at least 30 days prior to such change. The written notice shall set forth the current address, the complete address of the proposed new location, the effective date of the relocation, and shall be accompanied by a postal verification of the address of the new location, payment of the fee specified in section 1.2 of Supervisory Policy G 1 of this Title and the original license certificate.
