21 N.C. Admin. Code 19 .0605 - ENROLLMENT PROCEDURES
(a) Every school
certified by the Board shall furnish to each student upon enrollment a signed
copy of the school contract with the student and a copy of the school handbook
which shall include the school curriculum as approved by the Board. The school
shall also furnish to each student upon enrollment a copy of the statutes and
rules governing electrologists or laser hair practitioners and the sanitation
standards issued by the Board.
(b)
Every school shall obtain from each student on enrollment a signed copy of the
student's permission to receive electrolysis or laser, light source, or
pulsed-light treatment. Any limitations on treatment shall be listed on the
permission form.
(c) Within 10
business days after each student's enrollment, every school shall furnish the
Board with the following:
(1) The name,
address, date of enrollment, telephone number and specification of day or
evening classes of each student, recorded on the school's stationery;
(2) A statement signed by the student stating
that he or she has received a copy of the statutes and the rules governing
electrologists or laser hair practitioners and the sanitation standards issued
by the Board and is cognizant of the fact that in order to qualify for an
electrologist or laser hair removal license, the student must meet the
requirements of
G.S.
88A-10 or
G.S.
88A-11.1.
