21 N.C. Admin. Code 46 .2004 - REQUEST FOR HEARING
(a) A person
aggrieved by a Board administrative action who has not received a notice of
hearing from the Board may file a request for a hearing.
(b) The request for hearing shall contain the
following information:
(1) the petitioner's
name and address;
(2) a short and
plain statement of the Board action that the petitioner challenges;
(3) a short and plain statement of the way in
which the petitioner has been aggrieved; and
(4) an explicit statement of request for a
hearing.
(c) In order to
preserve a person's rights with respect to a Board action, the person shall
file a request for hearing with the Board within 60 days after the person
receives notice of the Board action that the person challenges.
