Ohio Admin. Code 3701:1-40-04 - Communications
(A)
Except as otherwise provided, any communication or
report required by this chapter shall be filed in accordance with Chapter 3748.
of the Revised Code and rules promulgated thereunder. Documents pertaining to
license application or any license matter, unless otherwise directed in
writing, shall be submitted to the following address or by electronic
submission in a manner approved by the director:
"Ohio Department of Health
Bureau of Environmental Health and Radiation Protection
246 North High Street
Columbus, Ohio 43215."
(B)
If
permitted to submit via electronic mail, communications may be sent to the Ohio
department of health at:
"BRadiation@odh.ohio.gov"
