Okla. Admin. Code § 86:21-5-1 - Forms

Each application shall include the following documents:

(1) Application Form;

(2) Official transcript, mailed from the university or college;

(3) Internship/practicum Documentation Form;

(4) Three (3) Document of Recommendation Forms;

(5) Two (2) classifiable sets of fingerprints; and

(6) Fees.
