55 Pa. Code § 1153.42 - Ongoing responsibilities of providers
(a)
Responsibilities of
providers. Ongoing responsibilities of providers are established in
Chapter 1101 (relating to general provisions). Psychiatric outpatient clinics
and psychiatric partial hospitalization outpatient facilities shall also adhere
to the additional requirements established in this section.
(b)
Recordkeeping
requirements. In addition to the requirements listed in §
1101.51(e)
(relating to ongoing responsibilities of providers), the following items must
be included in the records of MA beneficiaries receiving psychiatric outpatient
clinic, MMHT and psychiatric partial hospitalization outpatient services:
(1) The treatment plan must include:
(i) The treatment plan goals.
(ii) Services to be provided to the
individual by the psychiatric outpatient clinic or partial hospitalization
outpatient facility or through referral.
(iii) Persons to directly provide each
service.
(2) As part of
the progress notes, the frequency and duration of each service provided shall
be included.
