S.D. Admin. R. 67:42:17:06 - Required reporting of an incident or a change in circumstances
A provider shall report to the department within twenty four hours any occurrence of:
(1) A change of address
or other major change in the operation or ownership of the program;
(2) A change in household size or
composition;
(3) Damage to or a
change in the condition of the facility or home;
(4) An investigation of the provider or
program employee by the division of child protection services or law
enforcement concerning any allegation of child abuse or neglect or action that
may prohibit the provider or employee from meeting background check eligibility
requirements;
(5) Any serious
injury to a child that requires medical or dental attention; and
(6) The death of a child related to a serious
injury that occurred on the premises of the child care program.
