S.D. Admin. R. 67:42:17:35 - Required child records
A provider shall maintain a record for each child that shall include:
(1) Child's name and date of
birth;
(2) Parent or guardian's
name and telephone number;
(3)
Emergency contact name and telephone number;
(4) Parent permission for emergency medical
treatment;
(5) Names of individuals
authorized to pick up child as needed;
(6) Health information including any
allergies or special needs;
(7)
Current immunization record; or name of school for a school-age
child;
(8) Medication permission as
needed;
(9) Attendance
records;
(10) Date of enrollment;
and
(11) Date enrollment ends.
Each required child record shall be reviewed and updated annually and made available to the department for verification of the contents.
