Written notice of entitlement to judicial review is to accompany the final
order to the appellant and/or recipient. The notice is to be to the effect that
a person who is aggrieved by a final order in a contested hearing has the right
to request judicial review within sixty (60) days after the entry of the
agency's final order thereon.
