(1) A provisional
license may be issued to an applicant who has applied for but has yet to take
the licensure examination upon compliance with all provisions of rule
0880-11-.05(2) except subparagraph (j), and submission of proof of having
applied to take the examination.
(2) Provisional licenses are valid for a
period of one (1) year from the date of issuance but may be renewed in
accordance with rule 0880-11-.09 upon receipt of a renewal form signed by both
the applicant and the supervising clinical perfusionist.
A provisional license becomes invalid and
must be surrendered upon either of the following:
(a) The one (1) year licensure period has
expired and the licensee fails to renew or fails to obtain the supervising
clinical perfusionists' signature on the renewal form; or
(b) The licensee fails any portion of the
