Tenn. Comp. R. & Regs. 1050-02-.08 - LICENSURE RETIREMENT AND REACTIVATION
(1) Licensure
Retirement
(a) Licensees who wish to retain
their licenses but not actively practice osteopathic medicine may avoid
compliance with the licensure renewal process by obtaining from, completing and
submitting to the Board Administrative Office an affidavit of retirement form
along with any documentation which may be required by the form.
(b) Upon successful application for
retirement of licensure with completion and receipt of all proper documentation
to the Board's satisfaction, the Board shall register the license as retired.
Any person who has a retired license may not practice osteopathic medicine in
Tennessee.
(2) Licensure
Reactivation - Any licensee whose license has been retired may re-enter active
practice by doing the following:
(a) Submit a
written request for licensure reactivation to, and receive a reactivation
application from, the Board Administrative Office, and
(b) Complete the application and submit all
applicable fees and penalties, and
(c) If requested, after review by the Board,
Board members, or Board consultant/designee, appear before the Board, Board
member, or Board consultant/designee for an interview regarding continued
competence in the event of licensure retirement in excess of two (2)
years.
(d) The Board may attach
other conditions as deemed necessary including, but not limited to:
1. Submit, along with the application,
documentation of successful completion of the continuing medical education
requirements provided in rule
1050-02-.12.
Such required education must have been obtained within the renewal cycle
immediately preceding the reactivation request.
2. Successful completion of the Special
Purpose Examination (SPEX) and/or the Comprehensive Osteopathic Medical Special
Purpose Licensure Examination (COMSPEX).
(e) At the Board's discretion, submission of
any document or set of documents required by this rule or submission of
verification of the authenticity, validity and accuracy of the content of any
document or set of documents required by this rule directly from the FCVS or
other Board approved credentialing service to the Board Administrative Office
shall be deemed to be submission of originals of those documents or sets of
documents by the issuing institution(s).
