16 Tex. Admin. Code § 92.95 - Reporting Requirements
A course provider must submit a monthly report to the department containing the:
(1) total
number of course completion certificates issued;
(2) name and county of residence of all
participants;
(3) location of the
court and county where the offenses occurred; and
(4) dates of course completion.
