37 Tex. Admin. Code § 211.21 - Issuance of Duplicate or Delayed Documents
(a) If an original license , certificate, or
other document was previously issued by the commission , a duplicate of that
document may, if the document is current and valid, be issued after:
(1) an application is submitted;
(2) a check of commission records verifies
original issuance and continuing validity; and
(3) payment of any required non-refundable
application fee.
(b) The
effective date of this section is March 1, 2001.
