Wash. Admin. Code § 110-05-0020 - Determination-Notice
(1) The department
will issue a written decision that states whether a CPI is granted or
denied.
(2) The written decision to
grant a CPI must be sent by first-class mail to the individual's last known
address and to their email address, when available.
(3) If the department denies a CPI, the
written decision must be sent certified mail, return-receipt requested,
first-class mail, and email, when available, and include the following
information:
(a) The reasons for the denial,
including identifying any information relied upon that was not provided by the
applicant;
(b) Copies of any
information relied upon that was not provided by the applicant or directions on
how to obtain copies of the information;
(c) Notice that the individual has a right to
request a review of the denial and instructions for requesting a review;
and
(d) A statement that the
individual must wait two years from the date of the denial to request a CPI
again, if the denial is not successfully appealed.
