Wash. Admin. Code § 296-133-100 - Contents of petition - General
Petitions for the certification, decertification, or amendment of certification of an employee representative of a bargaining unit shall contain the following:
(1) A statement
as to whether the petition is filed by a health care activities employee
organization, a health care activities employee or a health care activities
employer.
