Wash. Admin. Code § 308-48-141 - Application for examination and reciprocity
(1) Applicants for initial licensing as a
funeral director or embalmer must submit:
(a)
An application on a form prescribed by the board.
(b) Official sealed college
transcripts.
(c) Official national
board or other state board examination scores.
(d) A nonrefundable application fee as
defined in WAC
308-48-800.
(2) Applicants seeking reciprocity must
submit:
(a) An application on a form
prescribed by the board.
(b)
Official verification of out-of-state licensure from the out-of-state licensing
board.
(c) Official sealed college
transcripts, if licensed in another state for a period of less than five
years.
(d) A nonrefundable
application fee as defined in WAC
308-48-800.
