W. Va. Code R. § 61-12H-4 - Bulk Pesticide Storage Facility Registry
4.1. All product manufacturers or registrants
shall notify the commissioner prior to the initial bulk shipment to any
repackaging establishment within this state. This notification shall include
the name and address of each repackaging establishment receiving such delivery
and an indication whether or not the establishment has been authorized by the
registrant to repackage in bulk or non-bulk quantities or dedicated containers.
The commissioner may disclose the information obtained pursuant to this rule as
permitted by W. Va. Code '19-16A-5(h).
4.2. All pesticide storage facilities
operating in this state shall register with the department by March 1 of each
year on a form provided by the commissioner.
