W. Va. Code R. § 64-70-4 - Application Procedures
4.1.
Applications for grants to offset the cost of providing uncompensated primary
care services shall be submitted by May 1 of each year, unless another date is
announced by the director , and shall be on forms approved by the director . The
director may request appropriate documentation or clarification of the
application from the applicant.
