W. Va. Code R. § 64-54-8 - Quality Assessment and Performance Improvement

8.1. A hospice shall establish a quality
assessment and performance improvement program that ensures quality of care is
provided to all patients. The program shall monitor, identify and take
corrective actions to identified problems.
8.2. The hospice shall designate a person or
persons responsible for the quality assessment and performance improvement
program.
8.3. The designated person
or persons of the quality assessment and performance improvement program shall
have a written plan for the annual review and evaluation of all hospice care
and services.
8.4. The annual
review and evaluation shall include a representative of each hospice service
offered.
8.5. The annual review
shall be sent to the governing body for review.
