Wis. Admin. Code Examining Board of Professional Geologists, Hydrologists and Soil Scientists GHSS 1.04 - Change of address
A licensee shall notify the board of a change of name or address as required under s. 440.11(1), Stats. The notice shall include the person's or firm's former and new addresses and each license or certification number held.
