Wis. Admin. Code Office of the Commissioner of Insurance § Ins 57.25 - Consent to jurisdiction

Any person required to file consent to jurisdiction under s. 648.45(3), Stats., shall do so in a format substantially similar to the format and questions contained in form C of Appendix 1 to this chapter.
