2 AAC 90.130 - Release from liability

A release from liability, as provided for in AS 28.20.100, must

(1) clearly state the name of the person released;

(2) be signed by the person making the release; and

(3) be notarized by a notary public or witnessed by a division employee using the release form provided by the department.
