Ariz. Admin. Code § R11-1-136 - Reporting of Accidents and Injuries
A. Where, in or about a mining operation, an
accident occurs, it shall be reported promptly to supervisor in
charge.
B. The State Mine
Inspector's office shall be immediately notified of:
1. Any accident or injury to an individual
resulting in death, or which has reasonable potential to cause death;
and/or
2. An entrapment of an
individual for more than 30 minutes; and/or
3. An unplanned inundation of a mine by a
liquid or gas; and/or
4. An
unplanned ignition of a blasting agent or explosive.
C. The State Mine Inspector's office shall be
notified within ten days of any accident or injury to an individual that, in
the opinion of the attending physician, may result in the injured person being
incapacitated for regular work for at least one day beyond the day of
injury.
