Ariz. Admin. Code § R11-3-201 - Document Submittals
A. A
responsible party shall submit to the Inspector any document required under the
Act or this Chapter by certified mail with a receipt or hand
delivery.
B. If the document
submitted under subsection (A) is a reclamation plan, the responsible party
shall ensure an original plus three copies, including any attachments to the
plan or amendments, are submitted to the Inspector.
C. For all documents submitted under
subsection (A), the responsible party shall:
1. Sign and date the documents or ensure the
documents are signed and dated by an individual with legal authority from the
owner or operator; and
2. Include
the names and addresses of the owner and operator.
