Ariz. Admin. Code § R13-6-702 - Application for Certification As a Firearms-safety Instructor
A. To be certified
as a firearms-safety instructor, an individual shall provide the following to
the Department:
1. A completed application
form with the following information:
a. Full
legal name;
b. Home address,
including ZIP code and county;
c.
Mailing address if different from the home address;
d. E-mail address;
e. Social Security number;
f. Driver license number or state
identification card number and state of issuance;
g. Home, business, and mobile telephone
numbers;
h. National origin or
race, sex, height, weight, eye and hair colors, and date and place of birth;
and
i. A statement whether the
individual suffers from mental illness or has been adjudicated mentally
incompetent or committed to a mental institution;
2. A properly completed fingerprint card with
classifiable fingerprints, unless the individual is currently registered as a
firearms-safety instructor under A.R.S. §
13-3112;
3. The current evidence of completion
referenced in
R13-6-701(7) ;
and
4. The fees required under
R13-6-103.
B. If an individual is qualified
under R13-6-701 and complies with the requirements in this Section, the
Department shall certify the individual as a firearms-safety
instructor.
