Ariz. Admin. Code § R17-4-202 - Certificate of Title Form
A. The
Motor Vehicle Division (MVD) shall produce the Certificate of Title form on
tamper-resistant and counterfeit-resistant paper.
B. MVD shall provide space on the Certificate
of Title form for the following information:
1. Title information:
a. Title number;
b. Issue date;
c. Previous title number; and
d. State and date of previous
title.
2. Vehicle
information:
a. Vehicle identification number
(VIN);
b. Vehicle make, model,
year, and body style;
c. Fuel
type;
d. Odometer information;
and
e. Vehicle mechanical or
structural condition.
3.
Lienholder information:
a. Lienholder name and
address;
b. Lienholder customer or
federal identification number; and
c. Lien amount and lien date.
4. Vehicle owner's or owner's
legal designee information:
a. Name;
and
b. Mailing address.
5. Ownership change information:
a. Sale date;
b. Purchaser's name and address;
c. Odometer mileage disclosure
statement;
d. Seller's signature;
and
e. Seller's signature
certification.
6. Dealer
reassignment information.
7. Other
information as required by the Division for internal processing and
recordkeeping.
