B.
A school license renewal application packet shall
include:
1.
A renewal application, completed on a form approved
by the Department, including:
a.
A list of all principals, contracted personnel, and
employees of the school who are responsible for Arizona school operations if
there have been any changes since the last renewal or original application;
and
b.
The signature of all principals on the completed
application; and
2.
Payment of applicable license fees prescribed under
A.R.S. § 28-3415 or 32-2374, for each activity, branch, and professional
driver training school instructor.
