Ariz. Admin. Code § R18-2-514 - General Permit Compliance Certification
A. A compliance certification submitted by
the owner or operator of a stationary source covered by a general permit shall
be on a form provided by the Director and shall include the following
information:
1. The source's name, mailing
address, contact person and contact person phone number, permit number,
compliance reporting period, and physical address and location, if different
than the mailing address.
2. A
certification of truth, accuracy, and completeness signed by the facility's
responsible officer.
3. Process
information for the source, including design capacity, operations schedule,
hours of operation, and total production.
4. Method of documenting compliance and the
status of compliance with all recordkeeping, reporting, monitoring, and testing
requirements and all emission limitations and standards imposed in the
permit.
B. Upon
notification from the Director of the availability of a web portal to complete
and submit a compliance certification, the owner or operator shall complete and
submit all compliance certifications through the portal.
