Ariz. Admin. Code § R18-5-403 - Application for Approval
A. An
application for approval, prepared in duplicate on forms furnished by the
Department, shall be filed at the time the plans are submitted for approval.
The form shall be completely filled out unless indicated otherwise.
B. The distance to the nearest public water
supply main and to a sewer main of a municipal or community system shall be
given.
