Ariz. Admin. Code § R18-9-H649 - Class IV; Closure Requirements and Remediation
A. Closure.
1. Prior to abandoning any Class IV well, the
owner or operator shall plug or otherwise close the well in a manner acceptable
to the Director.
2. The owner or
operator of a Class IV well must notify the Director of intent to abandon the
well at least 30 days prior to abandonment.
B. Remediation. Injection wells used to
inject contaminated groundwater that has been treated and is being injected
into the same formation from which it was drawn are authorized by rule for the
life of the well if such subsurface emplacement of fluids is approved by the
Administrator or the Director pursuant to subsections (B)(1), (2) or (3):
1. Provisions for cleanup of releases under
CERCLA, or
2. The requirements and
provisions under RCRA, or
3. The
requirements and provisions under other applicable state laws for corrective
and remedial action.
