Ariz. Admin. Code § R2-12-810 - Requirements for Reinstatement of a Restricted, Revoked or Probationary Registration After the Specified Term of Discipline
A. Unless otherwise specified in a
disciplinary order imposing revocation, the disciplined registrant may, after
two years from the date of the disciplinary order, petition for the
reinstatement of its registration.
B. Unless otherwise specified in a
disciplinary order a PEO whose registration has been restricted or put on
probation the disciplined registrant shall, at the end of the restriction, or
probation, petition for the release from the conditions of restriction or
probation.
C. Unless otherwise
provided by a disciplinary order, an applicant who applies for reinstatement of
a registration after the specified term of restriction or revocation of the
registration shall:
1. Submit an application
for registration complete with all supporting documents as is required when
making an initial application for registration demonstrating the applicant
meets all current qualifications for registration and compliance with
requirements and conditions of registration reinstatement;
2. Submit a Petition for Release from the
imposed disciplinary order that documents that all conditions of reinstatement
and requirements for re-registration have been fulfilled;
3. Pay the established registration renewal
fee;
4. Provide documentation to
the Secretary of State to clearly demonstrate the applicant is statutorily
qualified to be reinstated to engage in offering PEO services; and
5. Pay all monies due.
