Ariz. Admin. Code § R20-5-1515 - Notice of Adverse Condition, Bankruptcy, Change in Ownership Status, or Change in Business Address
A. A
Self-Insurer shall notify the Division in writing within 10 days of any adverse
condition or material change that impacts or could impact the Self-Insurer's
ability to process and pay benefits required under the Act. When a Self-Insurer
provides notice to the Commission under this subsection, the Self-Insurer shall
provide a written proposal to correct the actual or potential adverse condition
or material change.
B. A Public
Entity Pool shall notify the Division within 30 days of receipt of any
notification from the Director of the Department of Insurance and Financial
Institutions according to A.R.S. §§
11-952.01(N) and
41-621.01(L).
C. A Self-Insurer
shall notify the Division in writing within 10 days of any bankruptcy filing
under federal law or insolvency proceeding under any state's laws.
D. A Self-Insurer shall notify the Division
in writing within 30 days of any change in the ownership status or business
address of the Self-Insurer.
