Ariz. Admin. Code § R21-5-415 - Provision of Information on a Placed Child
After selecting an adoptive placement for a child, and before placing the child with the chosen adoptive parent, the adoption entity shall provide the adoptive parent with all non-identifying information available on the child, including, without limitation, the following:
1. All records concerning the child's
medical, psychological, social, legal, family, and educational
background;
2. All records
concerning the birth parents' medical, psychological, social, legal, family,
and educational background;
3. The
medical and social background on the child's other immediate family members,
including siblings and birth grandparents;
4. The child's plan for adoption services, as
described in R21-4-413; and
5.
Information on adoption subsidy that may be available for the child.
