Ariz. Admin. Code § R21-6-214 - Application for License Reinstatement
A. "Reinstatement" refers to an action by OLR
to reactivate a license that has been expired or closed for less than one
year.
B. The licensing agency shall
submit an application for reinstatement using the same process as an
application for renewal licensure. As required for a renewal, the application
for reinstatement shall include a new home study.
