Ariz. Admin. Code § R21-7-117 - Voluntary Closure
A. An
applicant or licensee may voluntarily withdraw an application for licensure or
close the license by submitting written notice to the Department.
B. If there are no children placed or being
served by the licensee the withdrawal or closure shall be effective on the date
selected by the licensee or, if no date is selected by the licensee, the date
of notification to the Department.
C. If the licensee has a child in care, a
licensee shall notify the Department and the Placing Entity 30 calendar days
before the voluntary closure date.
D. The licensee's written notice shall
contain the following information:
1. The
reason for the closure; and
2. If
currently licensed, the date by which the agency will close.
