Ariz. Admin. Code § R21-9-209 - Amended License
A. An
adoption agency that seeks to change its name, address, or offices, without a
change in ownership, shall apply to OLR for an amended license at least 14 days
prior to the effective date of the change.
B. The application shall be in writing and
shall specify the information to be changed.
C. So long as the change does not cause the
adoption agency to fall out of compliance with the standards listed in this
Article and A.A.C. Title 21, Chapter 5, Article 4, OLR shall issue an amended
license.
D. The amended license
shall expire at the end of the adoption agency's current licensing
year.
