Ariz. Admin. Code § R21-9-226 - Recordkeeping Requirements: Adoptive Children
The adoption agency shall maintain a case record for each adoptive child. Except as otherwise provided in A.R.S. § 8-129, the record shall be divided into two sections as follows:
1. Non-identifying information as required by
A.R.S. & 8-129; and
2.
Identifying information which shall include:
a. Tapes, videos, or photos of the adoptive
child or birth parent;
b. Legal
documents and reports required for adoption;
c. Social, physical, mental, and educational
history of the child's birth family;
d. Social, physical, mental, and educational
history of the adoptive child; and
e. A summary of all action taken to prepare
the child for placement in the adoptive home.
