Ariz. Admin. Code § R21-9-227 - Recordkeeping Requirements: Adoptive Parents
The adoption agency shall maintain a case record for each adoptive parent. If the adoptive parent is a member of the same family as another adoptive parent, the adoption agency can maintain one file for the adoptive family. The file shall include:
1. Documentation showing that the adoptive
parent received the orientation described in
R21-5-403 ,
2. The adoptive parent's application for
certification,
3. The adoptive
parent's certification report and any recertification reports,
4. A copy or description of the
non-identifying information the adoption agency has provided to the adoptive
parent pursuant to A.R.S. §
8-129(A),
and
5. A summary of the adoptive
placement decision and the pre-placement and post-placement contacts with the
adoptive family and the adoptive child.
