Ariz. Admin. Code § R3-1-307 - Petition for Review of a Practice or Policy
A person may petition the Director to review a practice or substantive policy statement, as prescribed in A.R.S. § 41-1033, that the petitioner alleges to constitute a rule. The petition shall contain:
1. The name, address, and
signature of the petitioner;
2. The
representative capacity of the petitioner, if applicable;
3. The practice or substantive policy
statement at issue, identified by Department division, number, title, date, or
concise description;
4. A statement
describing with specificity why the petitioner alleges the practice or
substantive policy statement constitutes a rule; and
5. The date the petition is signed.
