Ariz. Admin. Code § R4-10-203 - Personal License or Registration Renewal

A. An aesthetician, cosmetologist, hairstylist, nail technician, barber, or instructor licensee or an eyelash technician registrant shall electronically submit an application for renewal to the Board on or before the licensee's or registrant's birthday every two years.

1. If a licensee's birthday falls on a Saturday, Sunday, or legal holiday, the licensee may file the renewal application on the next business day following the licensee's birthday.

2. A renewal application consists of:

a.1. A form provided by the Board that contains the licensee's or registrant's name, address, e-mail address, Social Security number, and signature;

b. A copy of a government-issued identification containing a photograph of the licensee;

c.2. If the documentation submitted at the time of initial licensure or registration did not establish citizenship in the United States or was not a non-expiring work authorization, documentation specified under A.R.S. § 41-1080 that the licensee's or registrant's presence in the United States continues to be authorized under federal law;
d.3. A statement of whether the licensee or registrant has changed the licensee's or registrant's name since the previous application and, if the name has changed, a copy of a legal document, such as a marriage license, divorce decree, or driver license showing the name change; and
e.4. The fee required in R4-10-102.

B. An establishment licensee shall annually postmark or electronically submit to the Board an application for renewal on or before the license renewal date.

1. If the license renewal date falls on a Saturday, Sunday, or legal holiday, the licensee may file the application on the next business day following the license renewal date.

2. A renewal application consists of:

a. A form provided by the Board that contains:

i. The establishment's name;

ii. The licensee's license number; and

iii. If the licensee is an individual or partnership, the signature and tax identification number of the licensee or if the licensee is a corporation or limited liability company, the signature of the authorized signer and the tax identification number of the corporation or limited liability company; and

b. The fee required in R4-10-102 .
