Ariz. Admin. Code § R4-10-B202 - Application for an Eyelash Technician Registration
An applicant for an eyelash technician registration shall submit to the Board:
1. The fee for an
initial personal registration required in
R4-10-102;
2. A passport-style photo of the
applicant;
3. An application, on a
form available on the Board's website, that provides:
a. The applicant's name, full mailing and
physical addresses, e-mail address, telephone number, Social Security number,
and birth date;
b. For a licensed
cosmetologist or aesthetician, one of the following:
i. If the applicant was licensed by the Board
as a cosmetologist or aesthetician before October 30, 2023, the license number;
or
ii. A copy of the provisional
registration required under A.R.S. §
32-519(A)(3) verifying successful completion of a Board-approved eyelash technician training
program;
c. For an
individual not previously licensed by the Board, one of the following:
i. A copy of any eyelash extension training
certificate of completion received before October 30, 2023; or
ii. A copy of the provisional registration
required under A.R.S. §
32-519(A)(3) verifying successful completion of a Board-approved eyelash technician training
program;
d. A statement
of whether the applicant has ever had an aesthetics, cosmetology, hairstyling,
nail technology, or instructor license suspended or revoked in any state of the
United States or foreign country; and
e. The applicant's signature verifying the
information provided is true and correct;
4. Documentation of two years of high school
or its equivalent as defined at
R4-10-101; and
5. Documentation specified under A.R.S.
§
41-1080 indicating the applicant's
presence in the United States is authorized under federal law.
