Ariz. Admin. Code § R4-12-201 - Application for a State Equivalent Examination or Embalmer Assistant Practical Examination
An applicant for a state equivalent examination or embalmer assistant practical examination shall submit an application packet to the Board that contains the following:
1. An
application form provided by the Board, signed and dated by the applicant, and
notarized that contains:
a. The applicant's
name, mailing address, telephone number, and social security number;
b. Any prior name or alias of the
applicant;
c. The applicant's date
and place of birth; and
d. The
applicant's height, weight, hair color, and eye color;
2. A photocopy of the applicant's high school
diploma or general educational diploma issued in any state;
3. If applying to take a state equivalent
examination, a photocopy of the diploma issued to the applicant upon graduation
from an accredited or provisionally accredited school of mortuary
science;
4. Two passport
photographs of the applicant, no larger than 1 1/2 x 2 inches, taken not more
than 60 days before the date of the application; and
5. The fee required by the Board.
