Ariz. Admin. Code § R4-38-113 - Chelation Therapy Practice Requirements
A. Before a licensee may practice chelation
therapy for other than the treatment of metal poisoning, the licensee:
1. Shall document completion of the
postgraduate education required in
R4-38-104(C)(2);
and
2. Submit to and obtain
approval from the Board of the informed patient consent form required by A.R.S.
§
32-2933(27).
As part of the documentation submitted with the informed patient consent form,
the licensee shall include a copy of the chelation therapy protocol.
B. A licensee shall ensure that
detailed records and periodic analysis of results on patients consistent with
the most recent informed consent and protocol on file with the Board are
maintained consistent with A.R.S. §
32-2933(27) and available for periodic review by a peer
review committee designated by the Board. The licensee shall ensure that
retention of patient medical and treatment records conform to the requirements
of A.R.S. §
32-2936.
