Ariz. Admin. Code § R4-6-205 - Change of Contact Information
A. The Board shall communicate with a
licensee or applicant using the contact information provided to the Board
including:
1. Home address and telephone
number,
2. Address and telephone
number for all places of employment,
3. Mobile telephone number, and
4. E-mail address.
B. To ensure timely communication with the
Board, a licensee or applicant shall notify the Board in writing within 30 days
after any change of the licensee's or applicant's contact information listed in
subsection (A). The licensee or applicant shall ensure that the written notice
provided to the Board includes the new contact information.
