Ariz. Admin. Code § R4-8-208 - Application for Visiting Professor Certificate; Extension of Visiting Professor Certificate
A. To
obtain a visiting professor certificate, an applicant shall submit to the
Board:
1. The application form required under
R4-8-203(A) and a signed verification that the information provided is
accurate, true, and complete;
2.
The fee required under
R4-8-106(A)(8);
3. Documentation of at least five years of
experience in the practice of acupuncture;
4. Evidence of skill and training in the
subject that the applicant will be teaching, including one of the following:
a. Documentation from a college or university
of experience, education, or other training in the subject the applicant will
be teaching;
b. Documentation of
experience in teaching the same or similar subject matter content within the
two years before the application; or
c. Documentation of one year of experience
within the last two years in the specialized area in which the applicant is
teaching; and
5. A
detailed plan outlining the duties of the visiting professor.
B. A visiting professor
certificate is valid for one year from the date issued. To extend a visiting
professor certificate for another year, the certificate holder shall, at least
30 days before the certificate expires, submit to the Board an application for
extension. An application for extension includes:
1. The renewal application form described in
R4-8-204(D)(1) including a signed verification that the information provided is
accurate, true, and complete;
2. A
letter on official letterhead from an official of the school of acupuncture at
which the visiting professor will be teaching requesting that the extension be
granted; and
3. The fee required
under
R4-8-106(A)(9).
C. The Board shall not extend a
visiting professor certificate more than twice.
